Program fee项目费用
$99 per month. Billing begins when your treatment begins. The fee covers your care coordinator, ongoing physician access, specialist referrals when needed, the wellness app, and refill and delivery management.每月 $99，自您开始治疗之日起计费。费用涵盖专属健康管家、医生持续跟进、必要时的专科转诊、健康管理应用，以及续药与配送安排。
Medication药费
Billed separately at the pharmacy's price. Careby does not mark up medication. Your coordinator confirms the total cost and any insurance coverage before you pay. The pharmacy provides its own receipt.药费按药房实价另行收取，康伴不加价。付款前，健康管家会与您确认总费用以及保险可报销的部分。收据由药房直接开具。
If no medication is prescribed如未开具药物
There is no charge.不收取任何费用。
Cancellation取消
Cancel at any time by contacting your coordinator. Cancellation takes effect at the end of the current billing month. The fee is paused during treatment pauses directed by your physician.您可以随时联系健康管家取消，取消自当月计费周期结束时生效。如医生安排暂停治疗，暂停期间不收取月费。
Optional services可选服务
Registered dietitian consultations at $150 per session. Blood panels through Careby Health Assessments.注册营养师咨询，每次 $150。血液检测可通过康伴健康检测安排。
Payment付款
Charges are made to the payment method you saved, after you approve the total with your coordinator.健康管家与您确认总额、经您同意后，才会从您保存的付款方式中扣款。
